Patient Support Assistant- Christ House
The Patient Support Assistant position offers valuable experience supporting holistic care for Christ House patients. This position involves assistance with patient activities, case management tasks, and other administrative duties. This role is ideal for those interested in social work, public health, or nonprofit careers. This position requires working some evenings, weekends, and holidays.
1. Patient activities and support (40%)
• Plans and coordinates activities and events for Christ House patients that support their medical, social, spiritual, and recovery needs
• Coordinates with outside volunteers and Volunteer Manager on additional patient events and activities
• Encourages patient participation in Christ House activities and records attendance
• Leads and facilitates weekly community meetings for all patients in collaboration with medical staff and chaplain
• Provides additional support on holidays, such as Thanksgiving or Christmas, by assisting with events, covering nurses’ station, or other duties as assigned
• Escorts patients on appointments as requested
• Provides rotating coverage at the front desk or nurses’ station during lunch and as needed
2. Case management support (40%)
• Provides general support to case management at Christ House and Kairos House, including but not limited to verifying and applying for appropriate benefits (health insurance, social security, disability, etc.), coordinating appointments with clinical staff, developing and maintaining case management databases, and compiling statistics
3. Other departmental support (15%)
• Offers assistance to the Development Office as requested through compiling patient stories and assisting with other events
• Assists departments with collecting statistics and data entry throughout the year
• Provides support to Administration, Volunteer Services, Kairos, and Food Services as needed
• Participates in the life of the Christ House community by attending events and activities
4. Other duties as assigned. (5%)
